ACTIVITIES ASSISTANT – Per Diem. Hours and days vary; some evenings, weekends and holidays. Assist in the implementation of recreational programs for residents in the health center. Experience working with older adults in a long-term care setting. Must demonstrate patience to work with residents at all levels. Ability to initiate resident activity in both a group setting and individual basis. Ability to read aloud to large sized groups. Applications accepted until position is filled.
CNA - Part-time, 16 hours/week, 6:00 am – 2:30 pm, every other Saturday and Sunday plus one additional shift during the week. Must have current NYS certification as a CNA. Good communication skills and a desire to work as a part of a team providing quality hands on care to elderly residents with range of physical and cognitive deficits. Experience with medication assistance preferred. Ability to stand/walk most of the shift; ability to push, pull, lift and transfer residents weighing up to 250 pounds. Assistance with activities of daily living (including medication assistance) for residents in an adult home and skilled nursing environment. Applications accepted until position is filled.

CNA - Per Diem. Must be NYS certified. Certification in CPR and First Aid helpful. Ability to communicate
well with residents and families is essential.
HOME HEALTH AIDE – HOME CARE - Per Diem, work days and hours vary on a case by case basis. Must have current Home Health Aide certification from a NYS approved program. Dual certification as a Certified Nursing Assistant desirable. Promotes and restores residents’ quality of life by providing nursing care as determined by the needs of the residents and their individual plans of care. Ability to stand/walk most of the shift;
ability to push, pull, lift and transfer residents weighing up to 250 pounds. Good communication skills with residents, families, staff and visitors essential. Must be willing to work as part of a team in Assisted Living and Independent Living areas as well as Skilled Nursing Facility if CNA certified also. Flexible hours of availability desirable. LPN – Per Diem. Must have current licensure by the State of New York as a Licensed Practical Nurse. Experience in long term care setting preferred, successful candidate will also possess good observation, documentation and communication skills as well as a strong desire to work with elderly residents in a team centered environment. Familiarity/expertise in using electronic medical record preferred. Carries out MD orders for medications. Notes condition changes in residents and reports to nursing supervisor and MD as appropriate. Assists staff with care needs of residents including bathing, dressing, toileting and transferring. Checks physician orders for accuracy. Works with interdisciplinary staff to ensure that residents are functioning at their optimal level of independence. RN – Full-time, 32 hours/week, 10:45 pm – 7:15 am, every other Friday and Saturday night plus 3 additional shifts during the week. Must have current licensure by the State of New York as a Registered Nurse. Experience in
long-term care setting preferred. Successful candidate will also possess good observation, documentation and communication skills as well as a strong desire to work with elderly residents in a team centered environment. Successful candidate will also display leadership skills. Carries out MD orders for medications and treatments. Notes condition changes in residents and reports to Nurse Manager and MD as appropriate. Assists staff with care needs of residents including bathing, dressing, toileting and transferring. Works with interdisciplinary staff to ensure that residents are functioning at their optimal level of independence. Applications accepted until position is filled.
RN - Per Diem. Must be NYS licensed RN. Experience preferred. Ability to communicate well with residents and families.
